Thank you for your interest in one of our Golden Doodle puppies.  We request that you fill out
this application and we visit with you over the phone before taking a deposit.  We ask for a
$200.00 deposit, in the form of a personal check or money order for payment.  Deposits are
non-refundable and are applied to the purchase price. The remaining balance is due on February
16th, 2011 (earliest puppy pickup date). Once we receive your application form, we will contact
you to discuss our available puppies and help you choose a temperament best for you.

The prices for our F1b Golden Doodles puppies are; Female $700.00, Male $650.00

If you become unable to care for this dog, we request that you return the dog to us. We
never want one of our puppies/dogs to end up in a shelter or without a home.  If you can
find a good second home feel free to do so and just keep us advised. Otherwise please
