PUPPY CONTRACT & HEALTH GUARANTEE
This agreement is between (Seller) David Freeman
and

It is agreed between Seller and Buyer as follows:
CONDITIONS OF SALE
HEALTH GUARANTEE
Your Golden Doodle puppy has been bred from temperamentally sound parents. The Seller guarantees your
puppy to be free from diseases for 3 days after puppy leaves our premises. Puppy will leave our facility
clean, in good general health, and with no physical defects. Your puppy has been examined by a veterinarian
and inoculated with the appropriate vaccinations. Stool checks have been done on at least one occasion, and
the puppy has been treated if parasites were present.
CONDITIONS OF HEALTH GUARANTEE
The Buyer must take their puppy to their veterinarian within 3 days of receiving the puppy for
examination, testing and treatment for parasites if necessary, and to schedule ongoing
immunizations. Failure to have the puppy examined by veterinarian within 3 days of date puppy was
received by Buyer will void the health guarantee. Proof must be submitted to Seller that puppy has
been examined within the time specified. If the Buyer’s veterinarian finds the puppy to be in poor
health, a written description of the problems must be provided dated within 3 days of receipt. Upon
receipt of documentation from Buyer, Seller will refund the price of puppy or given a replacement
puppy, the choice to be determined by the Seller. Transportation costs will not be refunded and are
not included in this guarantee. Cost of exams and tests are responsibility of Buyer.
Buyer understands that stress related diarrhea is common and treatable. We do not consider
external or internal parasites an illness, as they are common in puppies. At times, coccidiosis can be
caused from the stress of the new home/environment. We recommend that the puppy should be
taken to the vet if diarrhea is noticed.
The Buyer agrees to keep their puppy at home and away from other puppies, public places, parks and
will use care in allowing others from outside their home to pet or touch the puppy. This is to protect
the puppy from exposure to Parvovirus and other infectious illnesses. Buyer will not enroll the puppy
in obedience training classes until all immunizations are completed – usually at 16 to 20 weeks of age,
according to their veterinarian’s schedule.
PUPPY BEHAVIOR/TEMPERAMENT
Buyer understands that all puppies exhibit certain undesirable behaviors (nipping, chewing, barking,
marking, etc). Buyer is responsible for appropriate training of puppy to correctly teach acceptable
behaviors, and is responsible for proper socialization of puppy.
MAINTENANCE
Buyer agrees to maintain this dog in good health, provide routine preventative health care including, but not
limited to, inoculation, internal and external parasites and prevention medication. Veterinary records must
be provided to breeder if requesting refund or replacement. Buyer must keep growing puppy at a healthy
weight appropriate for its size; must prevent harmful exercise that strains or negatively impacts joints;
must feed their puppy a nutritious and quality brand of puppy food that is specifically formulated for large-
breed puppies (for standard sized Golden Doodles).
Seller encourages Buyer to Spay/Neuter the puppy between 7 months and 1 year of age.
RETURN AGREEMENT:
If at any time the Buyer can no longer retain possession of this Golden Doodle, he/she will not give away, or
otherwise dispose of this puppy without first contacting the Seller, who will either accept the puppy on a
free return basis or assist the Buyer in finding a suitable alternative home for the puppy. This does not
imply a financial commitment on the part of the Seller.
OTHER PROVISIONS:
This puppy must not be resold to any owner, agent or individual connected with a pet shop or wholesaler of
puppies. This puppy is not to be turned over to any animal shelter or humane society.
Upon execution of this contract all veterinary care and its associated costs are the sole and exclusive
responsibility of the Buyer. Other than the above limited guarantee, the Seller assumes no financial
responsibility once this puppy has left Seller’s premises. Under no circumstances shall the Seller be liable
for any consequential, incidental or special damages resulting from or in manner related to the puppy.
FULL AND FINAL AGREEMENT
The parties agree that this writing represents the entire Agreement between them and that no other
representations, either oral or written have been made regarding the dog described above. The parties
further agree that no changes in this contract shall be binding upon either of them without written
modifications signed by both parties.
This agreement pertains to one F1b Golden Doodle Puppy, whelped on December 22nd, 2010
